We are seeking a compassionate, detail-oriented individual to fill a dual role as a Case Manager and Program Nurse. This position requires a client-focused professional who is empathic, flexible, and skilled in interpersonal relationships and communication. The role involves both in-office and in-home duties, ensuring that clients receive personalized care that supports their health, safety, and independence. You will be responsible for assessing client needs, developing care plans, coordinating services, and overseeing healthcare components, while working collaboratively with other professionals and caregivers. We are seeking full-time CNA’s to join our team. How will you spend your day? Our DayBreak CNA’s work to provide safe therapeutic activities focused on providing safe, engaging interactions, in a supportive environment. All while having fun with our participants. The right candidate must hold and maintain a current Oklahoma CNA License, the ability to lift at least 50 lbs., push 200 lbs., and walk or stand 90% of the time, while working. Good hearing and vision. Upon hire, will obtain/maintain current certifications in: First Aid, CPR, and Washington County Food Handlers. No cost to you!
