Elder Care is seeking a compassionate ADvantage Case Manager to help aging and disabled individuals live independently and safely. This role involves assessing client needs, developing personalized care plans, coordinating services, and providing ongoing support to ensure quality care. Candidates should have a bachelor’s degree, at least one year of experience with aging or disabled populations, a valid driver’s license, reliable transportation, and a willingness to travel (mileage reimbursed). Full-Time DayBreak Adult Day Health CNA

Activity Leader (CNA)

We are seeking a full-time CNA to join our team.  How will you spend your day? Our DayBreak CNA’s work to provide safe therapeutic activities focused on providing safe, engaging interactions, in a supportive environment. All while having fun with our participants. The right candidate must hold and maintain a current Oklahoma CNA License, and the ability to lift at least 50 lbs., push 200 lbs., and walk or stand 90% of the time, while working. Good hearing and vision. Upon hire, will obtain/maintain current certifications in: First Aid, CPR, and Washington County Food Handlers. No cost to you!
