Elder Care is seeking a compassionate ADvantage Case Manager to help aging and disabled individuals live independently and safely. This role involves assessing client needs, developing personalized care plans, coordinating services, and providing ongoing support to ensure quality care. Candidates should have a bachelor’s degree, at least one year of experience with aging or disabled populations, a valid driver’s license, reliable transportation, and a willingness to travel (mileage reimbursed).
