Intake Coordinator

Key Responsibilities

Full-Time DayBreak Adult Day Health CNA
Activity Leader (CNA)
Elder Care is seeking a full-time Certified Nursing Assistant (CNA) to join our DayBreak team. If you enjoy creating joyful, engaging experiences for older adults, this role is for you.
What You’ll Do
- Lead safe, therapeutic, and fun activities for DayBreak participants
- Build meaningful relationships while supporting each participant’s wellbeing
- Contribute to a positive, team-oriented environment
Requirements
- Current Oklahoma CNA license
- Ability to lift 50 lbs., push 200 lbs., and stand/walk for 90% of the workday
- Good hearing and vision
- Willingness to obtain/maintain certifications in First Aid, CPR, and Washington County Food Handlers (provided at no cost to you)
- Monday–Friday daytime schedule—no evenings or weekends

Qualifications

Social Worker (LMSW)
Social Worker
Elder Care is seeking a compassionate, team-oriented Licensed Master Social Worker who is dedicated to supporting older adults in a community-based setting. This role is a key member of our interdisciplinary care team and plays an essential part in helping aging adults remain healthy, supported, and connected.
Key Responsibilities
- Participate in assessments, care planning, daily care updates, and continuous quality improvement efforts.
- Serve as a primary liaison for participants, families, and internal/external service providers to ensure coordinated, person-centered care.
- Complete comprehensive psychosocial assessments and histories to support individualized care plans.
- Provide individual and family counseling, facilitate group support activities, and offer crisis intervention and advocacy when needed.
- Coordinate transitions of care, including hospital and nursing facility discharges, to ensure safe and seamless returns to home or community settings.
- Maintain accurate, timely documentation and support financial eligibility, benefits enrollment, and recertification processes.
- Participate in community outreach and staff meetings while remaining current on HIPAA/HITECH compliance and best practices through ongoing training.
Qualifications
- Licensed Master Social Worker (LMSW) required.
- At least one year of experience working with older adults, preferably in a community-based or long-term care setting.
- Strong communication skills, empathy, and a genuine commitment to serving frail and vulnerable seniors.
- Ability to thrive in a collaborative, fast-paced care environment.
