Elder Care provides Case Management Services through the Advantage program.
The Advantage Medicaid Waiver program provides alternatives to placement in a nursing facility for eligible adults. Members must meet income requirements and be nursing facility level of care to qualify through Oklahoma Human Services.
The program helps older adults and adults with disabilities stay independent and live at home safely. To learn more about our program, email us at AdvantageCM@AboutElderCare.org or call us at (918) 336-8500.
If you or your loved one needs medication management, nursing services, or a trained ear to accompany you or your loved one to medical appointments and procedures, our Care Management program may be just what you need.
Care Management works closely with our Brighter Living program and provides a variety of tailorable services. Care Managers visit the client and/or caregiver in the home to perform an initial assessment. Based on the assessment of the individual’s mental and physical capabilities, the home environment, finances, and social support system, a care plan is customized to meet the needs of your loved one. The Care Manager works with the team of health care professionals in the community to assure delivery of services designed to keep the individual in the home safely and comfortably. Care Management may be short-term care or long-term care depending on the condition of your loved one, the family’s concerns, and the support system available.
