Do you feel like you are losing your balance?
Falls in seniors over the age of 65 is a serious health risk and can threaten their independence. Elder Care’s Physical Therapy department offers free balance assessments called “Stay on Your Feet.” These assessments only take 10-15 minutes to perform in the clinic. Those who participate will receive information on fall prevention, physical activity and home modifications. If physical therapy is indicated, we will work to set up a more comprehensive evaluation as well.
Please contact Elder Care's Physical Therapy Department at 918-766-0391 or email Josh Lindblom at email@example.com to schedule your free "Stay on Your Feet" appointment.
What is Direct Access to Physical Therapy?
Many insurance providers, including Medicare, have allowed for a physical therapy evaluation to be performed without referral for years. However, under our state practice act, we could not provide any skilled treatment following that evaluation without permission from your physician. Now, Direct Access to Physical Therapy empowers you with the freedom to participate in a comprehensive evaluation PLUS appropriate treatment from a licensed physical therapist for up to 30 days without a referral from your physician. If care will extend beyond 30 days, physician recommendation will be required at that point. While the majority of our clients continue to arrive via the traditional referral route, Direct Access allows you to receive treatment in a timelier manner. YOU decide where you would like physical therapy.
Choose Excellence, Choose Elder Care.
What can I expect from my sessions?
On the first day, a licensed physical therapist will perform a comprehensive evaluation to understand your problem and how it is affecting your daily life. Family members are welcome to accompany clients and add details or support if you wish. During this evaluation, your therapist will establish an individualized plan of care, as well as the expected goals.
Each subsequent session will include one-on-one care with your therapist for up to an hour. Treatment duration is different for each individual. However, the average treatment cycle is two to three visits per week for one month.
Re-evaluations are performed every 10 visits or 30 days (whichever comes first) until the client has met all of their functional goals or is ready to transition to our Aftercare Program.
Are there any additional benefits?
Yes. Elder Care physical therapy offers our Aftercare Program exclusively to our physical therapy clients. Following skilled physical therapy discharge, the Aftercare Program allows clients to continue to utilize the gym and all of the fitness equipment to extend the benefits of their treatment. Our therapists supply each client with an individual exercise routine for ongoing maintenance of the strength and balance gained in therapy. Long-term, continued maintenance therapy is the key to remaining physically active and independent at home. There are two ways to participate in our Aftercare Program.
Independent Aftercare does not include direct supervision or assistance from a staff member and may be completed at your convenience during our regular business hours from 8 a.m. to 12 p.m. and 1 p.m. to 5 p.m., Monday through Friday. However, our therapy staff is always available to answer questions that you may have regarding the exercise program or equipment. This program requires pre-payment of $25 per month.
Assisted Aftercare includes scheduled individual sessions with a staff member, not a licensed therapist. The staff will assist or direct you through your exercise program that was established by your physical therapist. Your program may be adapted at the discretion of the trained staff member after consultation with the therapist. This program requires a pre-payment of $240 for 12 scheduled sessions, usually 2 or 3 times per week.
