Elder Care's mission is to ensure that mature adults live happy, healthy independent lives by linking them with the vital services necessary to maintain maximum independence in their home environments.
Through our programs we offer a broad range of services to seniors and their caregivers including: case management, adult day health services, caregiver support groups, home care services, specialized primary healthcare, physical therapy, and a social networking group.
Senior Strategies Seminar
Elder Care will close out the 2017 Senior Strategies Seminar Series on September 26 with Mindi Henry, APRN-CNP, ACHPN and her presentation titled, “Palliative Care – what is it and why does it matter?” Learn about how palliative care can make a difference when your loved one suffers from a chronic or terminal illness. Henry will share insight and understanding to a subject unfamiliar to many.
The seminar is open to the public and will be held 9:00am-10:00am at Elder Care, 1223 Swan Drive. Registration is recommended for these free seminars to ensure seating is available. Please call 918-336-8500.
Flu Shot Clinic
Following the Senior Strategies Seminars, Elder Care's annual Flu Shot Clinic will begin on Saturday, September 30 and continue each Tuesday in October. Times are 9am-noon and 1-4pm each flu shot clinic date.
Medicare Part D Counseling
Elder Care is offering free counseling assistance with Medicare Part D enrollment beginning October 16 through December 7.
Elder Care has four certified Medicare Part D counselors who will be available by appointment only. The appointments will typically last 45 minutes. Any interested participant will need to bring a current Medicare card, list of current medications, an identification card and if interested in applying for extra assistance to help pay for drug costs, a statement of current income and asset information.
Seniors should also ensure they are reading all mail they receive regarding current prescription drug plans. Some plans may not be offering the same services in 2018, which could result in having no plan at all as of January 1, 2018.

Elder Care's Golden Opportunities Program named Best Practice of the Year in the state by Leading Age Oklahoma
Elder Care’s Golden Opportunities program received the 2015 Best Practice of the Year Award by Leading Age Oklahoma. The Best Practice of the Year Award is given to an organization which displays outstanding initiative in the provision of services for older adults including a practice that demonstrates significantly improved access to services that enhance their quality of life and a practice that is unique to the organization and would be worthy of replication in other organizations.

Elder Care received DHS Aging Services' Program of the Year Award for Golden Opportunities. Our Golden Opportunities program received this award for making a significant contribution in service to older Oklahomans and their families.

Elder Care received the title of “Oklahoma’s top Nonprofit” at the Oklahoma Center for Nonprofits ONE Awards. Elder Care was first awarded the winner in the Seniors category and received the Anne and Henry Zarrow Foundation/Herman Kaiser Foundation Award. Then later in the evening, Elder Care was announced as the 2014 Top Nonprofit in the state of Oklahoma winning the Charles Schusterman Award for Excellence.

The Partnership Conference for Oklahoma Families Partnership Award. The Award was given to our DayBreak Adult Day Health program for their partnership with the Green Country Pilot Club. The Pilot Club has been a wonderful partner for our DayBreak program by helping us to purchase computer programs and fiber optic light tubes. ___________________________________
